General Guideline Principles for Gabapentin / Pregabalin

The New York State workers compensation board has developed these guidelines to help physicians, podiatrists, and other healthcare professionals provide appropriate treatment for Gabapentin / Pregabalin (Short Term) for CRPS.
These Workers Compensation Board guidelines are intended to assist healthcare professionals in making decisions regarding the appropriate level of care for their patients with ankle and foot disorders.
The guidelines are not a substitute for clinical judgement or professional experience. The ultimate decision regarding care must be made by the patient in consultation with his or her healthcare provider.
Gabapentin / Pregabalin (Short Term) for CRPS
Gabapentin / Pregabalin (Short Term) for CRPS is recommended to treat moderate to severe cases of CRPS when previous treatments have failed to bring symptoms under control.
Indications: CRPS in which various measures of symptom control, such as strengthening exercises, aerobic training, other types of exercises, NSAIDs, physical rehabilitation therapy, bisphosphonates, clonidine, and tricyclic antidepressants, are ineffective. Should be used in conjunction with a functional restoration NYS WCB MTG – Complex Regional Pain Syndrome 43 program to speed up program progression for the four weeks that the medication exhibits some efficacy. Beyond the first course, no further therapy is advised.
Frequency/Dose/Duration: One recommended dosage is 600 mg of gabapentin three times each day for days five to 21 after taking 600 mg twice daily for two days. Increased dosage should
